The Hazel Park Schools’ Nursing Office is planning for a successful 2024/25 school year. We are collecting medical plans for those students who have medical needs or concerns during school hours. If your student has any of the medical conditions listed below, please print the appropriate medical forms (located at the bottom of this webpage) and have your student's doctor fill them out. Completed forms may be emailed to the district school nurses, faxed to our nurse line, or dropped off at your child’s school. During the summer, completed plans may be emailed, faxed, or dropped off at the Hazel Park Schools Administration Building at 1620 E. Elza Avenue. Hard copies of medical plans are also available to be picked up at the Hazel Park Schools Administration Building. 

Medical Conditions:

⇔ Allergies (food, environmental, etc.)
⇔ Heart conditions
⇔ Diabetes
⇔ Migraines/Headaches
⇔ Multiple Sclerosis
⇔ Seizures
⇔ Stomach issues (Crohn’s disease, constipation, IBS, acid reflux, etc.)

Any student who requires medication during school hours
