The Hazel Park Schools’ Nursing Office is planning for a successful 2024/25 school year. We are collecting medical plans for students with medical needs or concerns during school hours. If your student has any of the medical conditions listed below, please print the appropriate medical forms (located at the bottom of this webpage) and have your student's healthcare provider fill them out. Completed forms may be emailed to the district school nurses, faxed to our nurse line, or dropped off at your child’s school. Hard copies of medical plans are also available to be picked up at each school building.

⇔ Diabetes

⇔ Migraines/Headaches

⇔ Multiple Sclerosis

⇔ Seizures

⇔ Stomach issues (Crohn’s disease, constipation, IBS, acid reflux, etc.)

Honor Community Health (in partnership with Oakland County Health Division) is offering the following primary/preventative care services at Oakland County Health Division's North Oakland Health Center (Mondays and Wednesdays) and at South Oakland Health Center (Tuesdays, Thursdays, and Fridays):

♦ Chronic Disease Management (high blood pressure, diabetes, asthma, etc.)
♦ Women's Health
♦ Pediatric Services
♦ Dental services (limited onsite/appointment only)
