Health Services

The Hazel Park Schools’ Nursing Office is beginning to plan for a successful 2023/24 school year. We are beginning to collect medical plans for those students who have medical needs or concerns during school hours. If your student has any of the medical conditions listed below, please print the appropriate medical forms (located at the bottom of this webpage) and have your student's doctor fill them out. Completed forms may be emailed to the district school nurses or dropped off at your child’s school. During the summer, completed plans may be dropped off at the Hazel Park Schools Administration Building at 1620 E. Elza Ave. in the front office. Hard copies of medical plans are also available to be picked up at the Hazel Park Schools Administration Building.

Medical Conditions:
► Asthma
► Allergies (food, environmental, etc.)
► Heart conditions
► Diabetes
► Migraines/Headaches
► Multiple Sclerosis
► Seizures
► Stomach issues (Crohn’s disease, constipation, IBS, acid reflux, etc.)
► Any student that requires medication during school hours

Honor Community Health (in partnership with Oakland County Health Division) is offering the following primary/preventative care services at Oakland County Health Division's North Oakland Health Center (Mondays and Wednesdays) and at South Oakland Health Center (Tuesdays, Thursdays, and Fridays):
