The Hazel Park Schools’ Nursing Office is planning for a successful 2026-27 school year. To ensure your student is properly cared for while at school, we are now collecting updated medical plans.
Does your student need a medical plan?
If your student manages any of the following conditions or requires any medication (prescription or over-the-counter) during school hours, a written medical plan may be required:
Medical Conditions:
- Asthma
- Allergies (food, environmental, etc.)
- Heart conditions
- Diabetes
- Migraines/Headaches
- Multiple Sclerosis
- Sickle cell disease
- Seizures
- Incontinence issues
- Stomach issues (Crohn’s disease, constipation, IBS, acid reflux, etc.
How to Complete Your Paperwork
1. Download and print the appropriate forms from the links at the bottom of this webpage.
2. Have your student's Healthcare Provider fill out and sign the forms.
3. Add your (parent/guardian) signature to the forms.
4. Submit completed forms via email, fax, or drop-off at your child's school.

*Note: Hard copies of all blank medical plans are also available for pickup at every school building office.
Summer Availability: Our schedule follows the school year calendar. District nursing staff are not available during the summer months.

Honor Community Health (in partnership with Oakland County Health Division) is offering the following primary/preventative care services at Oakland County Health Division's North Oakland Health Center (Mondays and Wednesdays) and at South Oakland Health Center (Tuesdays, Thursdays, and Fridays):
